To provide patients with a toolkit to move through the military transition process to help patients achieve optimal readiness for transfer of care to the VA. We aim: (1)To determine the feasibility and acceptability of a self-management intervention focused on successful transition to VA care upon discharge from active duty as measure by patient engagement and satisfaction. (2)To estimate effect sizes of the intervention on self-management skills including readiness for transition as measured by the IBD Self-Efficacy Scale (IBDSES) and the Transition Readiness Assessment Questionnaire [TRAQ]
